Medical Model 

hint: need to address medical necessity

hint: Progress reporting is necessary to show improvement [or lack thereof] with skilled OT

hint: Habilitative model for children, however, know the insurance guidelines for the length, number of allowable skilled OT treatments

hint: Most commonly used CPT code:  97535 (self care); 97110 (active units: which comprises therapeutic activities)

hint: Submit reports in a timely manner to avoid denial or delays in authorization

hint: Avoid highlighting sensory processing or integration as treatment & as a goal; as a goal unless it is tied to functional skill development or improvement, it may be denied

  • Address the limitations or weaknesses for goal development based on the result of standardized assessment or the objective measures-->develop or improve it as a functional skill

    • i.e. â€‹AROM BUE: Limited--> Goal: Increase AROM by 5% or 10 degrees--> to reach a cup at table height and bring it to mouth

  •  Be specific with the recommended frequency and duration

    • i.e. OT 2xwkx4wks; OT 2x30 mins a week for 12 weeks

  • Indicate the treatment modalities that is expected to be utilized 

    • i.e. Therapeutic activities; therapeutic exercises; manual therapy; TENS​

Educational Model

hint: Frequency and duration should NOT take the student away from the classroom time more than 50% 

hint: Change some quantifiers or qualifiers in the goal, if preferring to continue the same goal [a change has to be demonstrated]

  • Address the limitations or weaknesses for goal development based on the result of standardized assessment or the objective measures-->develop or improve it as a functional skill

    • i.e. BOT-2 Fine Motor Integration score--> below average-->Goal: improve scissor skills by cutting along 1/2" line​
